Forms for Competing

Submission Guidelines:

  1. Parent Permission Form: (For every tournament)
    This form is required for every tournament you participate in. Please ensure it is filled out completely and signed by a parent/guardian before the pre-tournament meeting.

  2. Student Code of Conduct: (Once every year)
    The Student Code of Conduct form must be printed, completed in ink, and submitted once before your first tournament. It is only required once for the entire year, so please ensure all sections are filled out accurately.

  3. Medical Form (For Travel Tournaments, only if you have a prescription medication):
    If your student requires prescription medication during a travel tournament, the medical form must be completed and submitted along with the Parent Permission Form. This is essential to ensure your student’s health and safety during the trip.

Submission Location:
All completed forms must be submitted as hard copies to Mrs. Basu in Room 2109 before any tournament.

Failure to submit these forms may result in ineligibility to compete.
