SRVUSD Tournament Forms

• Parent Permission Slip (needed for every tournament) and Student Code of Conduct (one-time) need to be printed and filled out completely in ink.

• Submit all forms as a hard copy prior to any tournament you compete in to Mrs. Basu in Room 2109.

• The medical form (one-time) needs to be submitted for prescription medications that a student needs for a travel tournament.

• Students who do not submit the forms at least two days before the tournament will be dropped.
